The 2020© McKenzie Assessment forms are updated to be in line with The McKenzie Institute's Educational Programming.
OPTP has access to the downloadable PDF Assessment Forms. You will also find Guidelines for Abbreviations and Terminology and for completing the MDT Assessment Forms that were developed by The McKenzie Institute and we encourage clinicians who are actively learning MDT to download them.
In addition, for those clinicians who are certified or started coursework before 2020 OPTP carries printed forms for purchase.
We would encourage all clinicians using MDT to review the updated Guidelines for Spinal Form Completion and endeavor to use the 2020© downloadable assessment forms at your earliest opportunity.
This Re-Assessment form was introduced on Part C and Part D courses primarily to assist course participants. However, MDT clinicians are encouraged to use it in daily practice.
We recommend that you download and save the PDF forms to a PC to print on demand without having to be connected to the internet. If your printer has a duplex (2-sided) option that is ideal. You may also photocopy the forms, and again, you are free to make as many copies as necessary to use with your patients. However, you may find this option more expensive than buying the pads from OPTP or the other McKenzie Global Limited agents who carry stock. Copyright of the forms remains with the McKenzie Institute International, licensor of McKenzie Global Limited. The actual form itself must not be changed in any way, however it will be acceptable for a hospital, etc. to incorporate their logo or name at the top of the form and requisite format for patient identification.
